Report of Injury by Telephone

To report a claim by phone, call (225) 389-5822 or (888) 884-5822, and you will be connected to our intake specialist, who gathers claim information.

We will put the information into our computer system, and once completed, your new claim will go to the proper claims handler. This relieves you of the paperwork associated with reporting a new claim. We will complete all necessary forms and handle filings with the Office of Workers’ Compensation.

Information you will need when reporting the claim by phone includes:

• Claimant's Information (First and last name, social security number, date of birth, gender, date claimant was hired) 
• Policy name and/or policy number  
• Date of accident 
• Description of accident and injury (including which body part(s) were involved)
• Name and contact number for person reporting the injury
